Active Minds Summer 2021 Week 1

Axis Education Trust is offering its famous holiday activity programme, Active Minds, between 2-25 August, Monday to Thursday weekly free of charge for 8-16 years old.
We are aiming to provide an effective, challenging, stimulating, and fun activity program offering a valuable respite for families and an opportunity to learn, try new activities, build friendships, relax, and most importantly have fun for children and young people which will bring a wide range of benefits to children, youth, families, and communities such as boosting academic performance, reducing risky behaviors, promoting physical health, healthy eating emotional and social development and providing a safe, structured environment for children and young people. 
There will be English and Maths booster activities, sports activities such as football, basketball, athletics, cycling and table tennis and performing arts. We will serve healthy food during the lunch break. 

Terms and Conditions

1)     I confirm that I will remind my son/daughter of the relevant safety procedures below that have been implemented by Axis Educational Trust in response to the CoViD-19 outbreak. If I have any concerns regarding the procedures, I will bring them to the attention of an Axis Educational Trust official in writing. I understand that these rules may be revised according to the government's advice at the time of delivery.

·       Entering the school from the Student entrance gate

·       Wearing a mask and keeping social distancing until put in bubble groups and instructed otherwise

·       Lining up in dedicated are for your bubble.

·       Allowing the temperature taken

·       Using the hand sanitisers before each activity

·       Not mixing with children of other bubbles during break time.

·       Not entering the toilet area if there are already two or more people inside.

·       Following the instructions and guidance before, during and after the activities

2)     I confirm that I will supply my child with the following:

·       Full Water Bottle

·       Hand Sanitiser

.       Trainers

3)     I confirm that I will ensure that my son/daughter has not been tested positive for CoViD-19 or shown any of the following symptoms AND has not been in contact with any person that has been tested positive for CoViD-19 or displayed the following symptoms in the 14 days before the activity (If you cannot confirm this to be the case, PLEASE DO NOT SEND YOUR CHILD TO THE ACTIVITY)

·       A new continuous cough

·       A high temperature

·       A loss of, or change in, normal sense of smell or taste

·       A sore throat

·       Shortness of breath

·       Feeling generally unwell

4)     I confirm that I give consent for Covid testing if required by the organisers.

5)     I understand that my child photo and video recording can be taken and used for reporting purposes.

6)     I agree that the information I am providing can be used in accordance with Axis Educational Trust's Data Policies that can be found at www.axiseducationaltrust.org. I understand that the information can be retained by those individuals deemed as appropriate and I can request that the information that is kept about me or my child can be viewed at any time.
